BY AGREEING TO THESE TERMS AND CONDITIONS YOU AGREE TO THE FOLLOWING:

I understand that the eyelash extension procedure requires synthetic eyelashes to be glued to my own natural eyelashes.

I understand that it is my responsibility to keep my eyes closed and remain still during the entire procedure until my eyelash technician addresses me to open my eyes.

I understand that I am required to follow the eyelash extension care sheet or aftercare which was also verbally explained in order to maintain the life of my eyelash extensions. 

I understand failing to following aftercare sheet and guidelines which were also explained verbally by my lash tech may result in poor retention, bacterial infections as well as refusal of service by Richelle’s Beauty Boutique. 

I agree to disclose any allergies and/or allergic reaction that I may have/had to latex, surgical tapes, CYANOACRYLATE (eyelash extension glue), Vaseline, etc.

I agree to disclose any eye disorders that I may have.

I agree that by reading and agreeing to this consent form, I release Richelle’s Beauty Boutique for any claims or damages of any nature.

I agree that I have read and understood this consent form.

I am of sound mind and fully capable of executing this waiver for myself.

I give Richelle’s Beauty Boutique permission to show my before and after photos of eyelash extensions to other potential clients as well as post the pictures to the company website and social media accounts.

I have read and completed the eyelash extension intake form and consent form in its entirety, and have answered everything to the best of my ability. I have been informed of potentially harmful or negative side effects that may be caused by the application and/or removal or eyelash extensions.

I confirm and agree that I wish to engage in the services of Richelle’s Beauty Boutique to apply my eyelash extensions.

STANDARD TERMS AND CONDITIONS

Change of Service:

If you require any change or additional services, these must be requested prior to the appointment to ensure ample time is reserved for any appointments which may require additional time. Request may be made with receptionist prior to check-in however change of service may not be guaranteed if tech is already booked. 

Performing fills on another technicians work who is not employed at Richelle’s Beauty Boutique. We do not fill on another techs work. Quality of work is not guaranteed and it is more work for your Richelle’s Beauty Boutique Tech to attempt to fill on a set done by (more often than not) an unqualified, inexperienced lash tech. Please book in for a full set if you are coming in from another lash salon as we will charge you for a full set without notice in the time which was booked for your appointment which will not be the appropriate time needed for a full set. 

Lash Lifts:

If you have had a lash lift within a month before your eyelash extension appointment, it is YOUR RESPONSIBILITY to let your eyelash technician know before applying your eyelash extensions. Richelle’s Beauty Boutique has the right to refuse your service and an additional fee of $100 will be applied to your final bill as your appointment will be considered a no show. Richelle’s Beauty Boutique can not perform satisfactory service on any individual who has had a lash lift in the past month.  In Order to Better Serve You:

ONLY THE CUSTOMER AND THE EYELASH TECHNICIAN ARE ALLOWED IN THE SERVICE ROOM WHILE SERVICES ARE BEING PERFORMED. 

Product purchases are non-refundable.

We do not accept cheques.

Gratuities and taxes are not included in our prices.

NO SHOW POLICY FOR APPOINTMENTS: 

Failure to not show up for an appointment without providing 24 hours notice of cancellation, arriving 10 or more minutes late will result in the forfeiture of the entire appointment. Missed/No Show services will be billed out 100%.

Please provide ample time to arrive to your appointment on time. Please factor in traffic and finding parking.

CANCELLATION, LAST MINUTE CHANGE TERMS & CONDITIONS:

If the appointment is cancelled LESS than 24 hours before the time of the appointment, a 50% charge to the customer which will need to be paid prior to rebooking.

You will receive 2 emails for your appointment. First email will be sent immediately after booking your appointment. Second email which is a Confirmation email will be sent 48 hours before the appointment. You may go to our online booking system and cancel your appointment at your convenience or email and call the salon. Should you choose to keep your appointment after the second email is sent, you have 24 hours to cancel to avoid any fees. 

You will not be rebooked until all fees are paid in full. 

SERVICE RETURN POLICY:

We do not give refunds on services rendered. Due to the nature of the service, please notify Richelle’s Beauty Boutique within 3 days of service date with any problem and/or concerns regarding your service. We will gladly correct the problem, as refunds will not be given.

Classic Full Set  $100.00

Classic Fill:

Two week classic fill (7-14 Days) -$50.00

Three week classic fill (15-21 Days) $70.00

21 Plus Days classic fill (22-31+ Days) $100.00     

Lash Lifts:

Lift & Tint

Lash Lift $50.00

Lash Lift and Tint $65.00

Waxing: $10.00-60.00 per service section. Ask for detailed price list
